कान्तिपुर सम्पादकीय सम्बन्धमा...


कान्तिपुरको आजको सम्पादकीय पढें। चितवन मेडिकल कलेजमा भएको पछिल्लो घटनाको बारेमा लेखिएको उक्त सम्पादकीयको आशय ठिकै होला तर उक्त घटनालाई जसरी उदाहरण दिएर त्यसले आफ्नो कुरा भन्न खोजेको छ त्यो चाँही मलाई उतिसारो ठिक लागेन।

"पानीको पोका भनेर बिरामीको दाँया मृगौला झिकेर गैरजिम्मेवार र लापरवाही प्रवृतिको नमुना पेश गरे।" "पानी जमेको डल्लो र मृगौलाबीचको अन्तर छुट्याउन नसक्ने चिकित्सकले कसरी शल्यक्रिया गर्ने अनुमति पाए?"

हो, कहिलेकाहि शल्यक्रिया गर्ने चिकित्सकले पनि त्यो अन्तर छुट्याउन सक्दैनन्। त्यसैले त चिकित्सा विधामा प्याथोलोजी/pathology र रेडियोलोजी/radiology भन्ने छुट्टै डिपार्टमेन्ट हुन्छ। नांगो आँखाले हेरेर सधैं सबै अंग र रोग पत्ता लगाउन सम्भव छैन, त्यसैले त अप्रेशन गर्नु अघि एक्सरे, भिडियो एक्सरे, सिटि स्क्यान, एमआरआई, आइभीयु जस्ता रेडियोलोजिकल जाँचहरु गर्ने गरिन्छ। यस्ता जाँचहरुले अप्रेशन अघि अप्रेशन गर्ने चिकित्सकलाई रोग र रोगको अवस्थाका बारेमा बढिभन्दा बढि जानकारी दिएर अप्रेशनमा सहयोग गर्छ। त्यस्तै कहिले अप्रेशन अघि नै र कहिले अप्रेशन गरेर निकालेपछि निकालिएको मासु वा तन्तुको प्याथोलोजिकल जाँच गर्न पठाइन्छ। विभिन्न प्रकारका केमिकलहरु प्रयोग गरेर शक्तिशाली माइक्रोस्कोपहरुबाट जाँच गरेर मात्रै निकालिएको तन्तुमा कस्तो प्रकारको रोग थियो, मेलिगनेन्ट/malignant (फैलन सक्ने क्यान्सर) वा बेनाइन/benign (नफैलने खालको क्यान्सर) हो, गरिएको अप्रेशन पूर्ण थियो वा फेरी दोहोराएर गर्नुपर्ने हो आदी भनेर थाहा हुन्छ। सामान्य अवस्थाको मृगौला भएको भए त एउटा कुरा, रोगले बिग्रेर पानीको डल्लो जस्तोमा परिणत भैसकेको मृगौलालाई अनुभवी सर्जनै भएपनि कसरी चिन्ने? जसरी विमान दुर्घटनामा क्षतविक्षत भएका शवहरुलाई आफ्नै आफन्तहरुले त चिन्न नसकेर डिएनए जाँचको भर पर्नुपर्ने हुन्छ, त्यसरी नै रोगले बिग्रेर सबै कुरा एउटै डल्लो भएको मृगौलालाई चिन्न नसक्नु सर्जनको कमजोरी हो जस्तो लाग्दैन।

अप्रेशन अघि राम्ररी भिडियो एक्सरे र सिटी स्क्यान गरिनुपर्थ्यो। यस घटनामा सिटी स्क्यानको लागि विरामीका आफन्तहरुलाई सुझाव दिइएको थियो तर आर्थिक अवस्थाका कमजोरीका कारण देखाइ त्यसलाई नगर्ने भनियो। र शुरुको भिडियो एक्सरे पनि एउटा रेडियोलोजिस्टले नभएर एक एमडिजीपीले गरेका थिए। म एमडिजीपीले भिडियो एक्सरे गर्नैहुन्न भनेर भन्दिन। हाम्रा गाँउघरतिर र कतिपय शहरी क्षेत्रहरुमा पनि एमडिजीपी र कहिले त एमबीबीएस डाक्टरले पनि केहि महिनाको ट्रेनिङको भरमा भिडियो एक्सरे गरिरहेका छन्।  उनीहरुले लामो दुरी पार गरेर शहरै पुग्नुपर्ने, धेरै पैसा खर्च हुने समस्याबाट केहि भनेपनि राहत दिएका हुन्छन्। अझ रेडियोलोजिस्टको अभाव भएका कति सरकारी अस्पातलहरुमा एमडिजिपिहरुले नै त्यो सेवा दिइरहेका हुन्छन। तर सम्झनुपर्ने के हो भने कम्लिकेटेड, जटिल अवस्थाहरुमा राम्रो डायग्नोसिस र रिपोर्टिङका लागि उनीहरुले रेडियोलोजिस्टको सहयोग लिने गर्नुपर्छ वा रेडियोलोजिस्टकोमा रिफर गर्ने गर्नुपर्छ, जुन बानीको हाम्रो समुदायमा कमी छ।

माथिको केसमा अप्रेशन गरेर निकालिएको त्यो पानीको डल्लोमा मृगौला पनि हुनसक्छ भनेर विरामीका आफन्तहरुलाई राम्ररी नसम्झाइनुमा गल्ति भएको हो, रोग र रोगको फैलावट र जटिलतालाई अप्रेशन अगावै पुनः रेडियोलोजिस्टसँग भिडियो एक्स रे र सिटि स्क्यान गरेर राम्ररी नहेरी विरामीका आफन्तको आर्थिक समस्यालाई ठुलो मानेर त्यहि अनुसार चल्नुमा लापरवाहि भएको हो। तर विरामी र उसको रोगको उपचारमा र गर्नुपर्ने अप्रेशनमा केहि गल्ति भएको छैन। गल्ति अप्रेशन अघि र पछि अप्रेशनका दौरान आइपर्नसक्ने र आइपरेका जटिलताका बारेमा विरामीका आफन्तहरुलाई राम्ररी समय दिएर नबुझाइनुमा भएको हो। तर त्यसलाई नेपालकै सबैभन्दा बढि बिक्री हुने दाबी गरेको पत्रिकाको पनि सम्पादकियमा राम्ररी कुरै नबुझी लेखिनु कत्तिको सम्वेदनशीलताको प्रर्दशन हो?

हल्लैहल्लाको देश हो नेपाल, तर त्यसमा पत्रकारहरु जस्तो जिम्मेवारी बोकेका मानिसहरुले त्यस्ता अनावश्यक हल्लाहरुलाई कम गरेर सम्वेदनशील क्षेत्र भनेर भनिएको स्वास्थ्य क्षेत्रमा बढ्दो अविश्वासको खाडललाई कम गर्न सहयोग गर्नुपर्नेमा हल्लाको पछि दौडेर सत्यतालाई झन बंगाउने काम गरेका छन्। त्यो हल्ला "उपचार गर्दागर्दै मृगौलै गायब", "मृगौला नचिन्ने सर्जन", "पेटको अप्रेशन गर्दा मृगौलासमेत झिकियो" हुँदाहुँदै साधारण जनमानसमा "विरामीको अर्कै रोगको अप्रेशन गर्दा नचिनेर गल्तिले राम्रै मृगौला झिकियो" वा अझ "अप्रेशन गर्ने चिकित्सकले विरामीको मृगौला गायब गरे" भन्ने सम्म बन्न गयो। यसले विरामीको अस्पताल र चिकित्सकहरु प्रतिको विश्वासमा कस्तो धक्का पुग्न जाला सहजै अनुमान लगाउन सकिन्छ।

अस्पताल र स्वास्थ्य क्षेत्र सम्वेदनशील क्षेत्र हो तर के यो चिकित्साकर्मीको हकमा मात्र लागु हुने कुरा हो र? के पत्रकार र पत्रिकाहरुलॆ यसको सम्बन्धमा लेख्दा यसले विरामी र चिकित्सक बीच घट्दै गैरहेको विश्वसनीयताको वातावरणलाई कत्तिको नकारात्मक असर पार्छ भन्ने कुरालाई ख्याल राख्नुपर्दैन? के त्यसमा चाँही उनीहरुले सम्वेदनशीलता देखाउनु उनीहरुको नैतिक जिम्मेवारी होइन? समाजलाई जागरुक बनाउनु, समाजमा भइरहेका नकारात्मक कुराहरुलाई सबैका अघि ल्याएर प्रस्तुत गरी त्यस्ता काम गर्ने, गराउनेहरुलाई हतोत्साहित पार्नु पत्रकारहरुको धर्म हो। यो मैले भनिराख्नुपर्ने कुरा हो जस्तो लाग्दैन। तर पहिलो बन्ने होडबाजीमा, अरु पत्रिका वा च्यानलभन्दा पहिले समाचार ब्रेक गर्ने कम्पिटिसनमा समाचारको गुदी कुरा नै नबुझी एक पक्षिय हिसाबले र आफ्नो सोचाइका अनुसारले समाचारलाई बंगाएर गलत अर्थ लाग्ने तरिकाले आम मानिसका सामु प्रस्तुत गरिनु कस्तो पत्रकार धर्म हो? सम्वेदनशील क्षेत्र भनेर कुर्लने अनि त्यहि क्षेत्रका बारेमा चाँही राम्ररी कुरै नबुझी दौडने अनि सबैलाई दौडाउन खोज्ने कस्तो सम्वेदनशीलता हो? हुन त "कागले कान लग्यो" भन्दा कान छाम्न छोडेर कागको पछी दौडने १०० मीटरका धावक जस्ता पत्रकारहरुको दौडने र दौडाउने बानी केहि समय अघिको "अनुजा काण्ड"ले पनि प्रस्टै देखाएको थियो। भलै कान्तिपुरले पछी त्यसमा क्षमा माग्यो। तैपनि पत्रिका र पत्रकार भनेका अझै पनि सबैभन्दा भरपर्दा माध्यम हुन्। उनीहरुको आवाज सबैले सुन्छन् र धेरैले पत्याउछन् पनि। त्यसैले त हामी उनीहरुको व्यवहारमा र रिपोर्टिङमा धावकियपन भन्दापनि सत्यता र सम्वेदनशीलताको आशा गर्छौं।

हो, हामीमा पनि कमजोरी छ। हामी मध्ये धेरै पैसाको पछि दौडेका छौं। विरामीलाई सेवा दिने भन्दा कसरी बढि भन्दा बढि विरामी हेर्ने र लगानी उठाउने भन्ने होडबाजीमा लागेका छौं। विरामी र उसका आफन्तसँग रोग र विरामीका अवस्थाका बारेमा राम्रोसँग एकछिन कुरा गर्न हामीसँग समय हुदैंन। विरामी र उसका आफन्तलाई रोगको बारेमा जानकारी दिएर उपचारका लागि फैसला उनीहरुलाई नै लिन दिने र त्यसका लागि उनीहरुलाई सहयोग गर्ने हाम्रो बानी छैन। नाफाकै लागि खोलिएका नर्सिङ होमहरुको त कुरै छाडौं, चिकित्सकहरुमा र चिकित्सक उत्पादन गर्ने मेडिकल कलेजहरुमा पनि व्यापारिकरणको भुतले छोएको छैन भन्दा गलत हुनेछ। तैपनि जानीजानी विरामीको लागि नराम्रो सोच्ने र नराम्रो गर्ने चिकित्सक त शायदै होलान्। आफ्नो क्षमताले भ्याएसम्म र अनुभवले जानेसम्म विरामीलाई उपचार दिन प्राय: सबै चिकित्सकले खोजेका हुन्छन्। यस्तोमा विरामी र चिकित्सकबीचको बचेखुचेको विश्वसनीयताको वातावरणलाई सुधार्नमा सहयोग गर्नुको साटो भएको पनि बिगार्ने गरी वास्तविकता नै नबुझी "कान्तिपुर" जस्तो पत्रिकाको सम्पादकीय लेख्नुले हामी चिकित्सकहरुको पत्रिका र पत्रकारप्रतिको विश्वासलाई हल्लाएको छ। अत: समाचार लेख्दा समाचारको सम्वेदनशीलता र सत्यता बुझेर त्यसलाई नबंगाई, अनावश्यक र गैरजिम्मेवारपुर्ण तरिकाले रंगीचंगी नबनाई लेखिदिनुहुन समस्त पत्रकार साथीहरुमा अनुरोध छ। तपाईंहरुको पनि आशय समाज जगाउने होला तर सत्यको बाङो अर्थ नलाग्ने गरी लेखेर त्यसो गरे कसो होला?

अन्तमा सम्पादकीयकै अर्को वाक्य सापटी लिन्छु, "यो अवस्थामा सरकार, स्वास्थ्य मन्त्रालय, मेडिकल कलेजहरु, अस्पताल व्यवस्थापन, मेडिकल काउन्सिल लगायत स्वास्थ्य क्षेत्रका नेतृत्वदायी संघसंस्थाहरु गम्भीर हुनुपर्छ। जनतामा चिकित्सा पेसाप्रति उत्पन्न हुदैं गएको डर र आशंकालाई चिर्न केन्द्रित हुनुपर्छ।" यसमा म सानो कुरा थप्न चाहन्छु, यो काममा पत्रकार र पत्रिकाहरुको पनि गम्भिरताको आवश्यकता छ र उनीहरु पनि यस काममा सम्वेदनशीलताका साथ हातेमालो गर्छन् भन्ने आशा राखौं।

  2. First things first. I have no idea if you are associated with CMC or not.

    Without generalising the above case, the overall quality of medical services (not necessarily the medical service provider) of our medical institutions, in the sense of gathering the relevant medical history of the patient and intimating the patient and their relatives on the nature and need of tests performed/required has been very very poor. The family, on one hand, neglect the relevancy of this communication and the medical personnels on the other hand, either are too 'busy' or are not sincere enough to share that extra time with the patients. This attitude from both, the patients and the doctors, results to the mess like this one. And this risk is even graver as our society idolizes and lionizes doctors and doctors easily fall prey to this notion.
    The crux of the problem lies in almost non-existence of standard operating procedures (non-compliance of it, wherever it exists) in the vital treatment and surgeries undergoing in our medical institutions. And chitwan is no exception. Unless standard procedures are designed and strictly followed, these incidents will only grow in future.
    Few months back, I was admitted to EMS ward in the CMC. Observing the hospital procedures, I had already sensed the risk of these kind of disasters. Everyday a new doctor would make a round and go through my case history and treatment, everyday a new doctor, where it should have been the other way round, a doctor visiting me everyday. I was in EMS ward, their best services ward and I was frustrated with repeating my case history to a new man everyday. This was bound to affect my treatment depending on the expertise of each doctor I had briefed to.

    CMC management should have responded to the wake up call when a patient died from asthma from the wrong treatment of TB few months back. They never took any steps to avoid such communication failures. And now this thing happened. Where they could have easily averted such failures by establishing a proper mechanism for the patient identification and follow-up process from the time of admission till the recovery. This doesn't need too much time, resource or expertise. A simple workflow can be designed and implemented within short span of time. All it need is swift management response, sense of responsibility and dedication to the work and patient in general.

    It still, is not too late. For a prestigious medical institution such as CMC, its high time they implement universally accepted standard medical procedures and setup a communication desk to properly intimate the relatives of the patient to the plan, need and possible consequences of any treatment they are undergoing. This might not completely prevent such incidents in the future, but it will certainly ease the pressure for the medical professional and the patients and their relatives can adjust their expectations from the professionals. This will reconcile the differences between the two parties.


    When our medical professional treat us as a subject of treatment and not as a living human, well, god save us.

  4. @Chasing_che...... i want to clear u one thing, that asthma patient didn't died due to wrong medication, he died because he didn't got medicine in time.Yes , its true he got wrong discharge sheet, but he hadn't taken medicne for tb.

  6. @princencess Paper reported the patient died of wrong medication. And there had been no clarification on the issue by the hospital management. How can we be sure that he died from non-availability of medicines.

  7. @Chasing_che: That's what i am saying in this article. 'Journalists writing about the incidents without knowing the truth about it.' We are also wrong in these cases by not clarifying the things publicly doing a press conference. Most of us think it's better settled with the families and not to make it a big public issue. In the incident that you are saying it was COPD (Chronic Obstructive Pulmonary Disease) patient, not an Asthma pt. Both COPD n Asthma are called 'दम' in nepali layman term. Nobody knows for sure what she died of. It could be due to complications of her disease itself, or due to delay in treatment or due to wrong medication. That's why the hospital management had tried to convince the family to help them to know the cause of death. But only compensation was highlighted.
    Anyway, what i want to say is most of us believe blindly what papers say, so they should behave responsibly in these kind of incidents. Because health sector is sensitive area and becoming more these days but we can't and shouldn't expect such behavior from health workers only. We need help and support from all of you.
